Registration Information
Step 1: Fill out a pre-registration form for your student. (Link Above)
Step 2: Schedule an In Person Appointment: At the end of the online form, you will be given a Sign-up Genius Link to schedule an in person appointment to complete your registration. An in person appointment is required.
Step 3: The following documents are required at the in person appointment.
Original Birth Certificate
Student's most recent Immunizations and Physical
Proof of Residency (see below)
Proof of Occupancy (see below)
Parent/Guardian's photo ID
Release of Student Records (for students in grades 1-12)
Copy of student's IEP/504 (if applicable)
Any prior student records from previous school (grades 1-12)
Residency Documents - 1 of the following documents is required.
Recent Mortgage Statement
Property Tax Bill
Mortgage Closing Documents
Copy of Signed Lease
Rental Agreement
Section 8 Agreement
Tenant at Will Document
Residency Certificate (notarized)
Occupancy Documents - 2 of the following documents are required.
Gas/Oil Bill
Electric Bill
Cable Bill
Landline Phone Bill (not cell)
Health Insurance Benefits (with full name & address)
Occupancy Affidavit (notarized)
All Scheduled In Person Appointments are held at: The Central Administration Office, 360 Lynn Fells Parkway, Melrose. (the entrance to Central Administration is located in the courtyard separating the high school and middle school)
