GRADES 6-8 (Middle School)

The following information is required in order to register your child in the Melrose Public Schools. These forms may be downloaded. Please bring the completed forms to the Melrose Veterans Memorial Middle School, located at 360 Lynn Fells Parkway, Melrose, MA 02176. Registration must be done in person and is not considered complete until all the required information has been received. Please contact the Guidance Office to schedule an appointment by calling 781-979-2100.
