Just a few Reminders for Parents:•To all parents/guardians: Please fill out the Confidential Student Health and Emergency Form and send in to school nurse. This form may be found at the Melrose School website and must be filled out every school year. •To all Kindergarten parents: A copy of a current doctor's physical and required immunizations with a lead screening test are now due on ALL KINDERGARTEN STUDENTS. Please forward to the school nurse, if you have not already done so.
•To all Grade 3 parents/guardians: A copy of a doctor's physical is recommended for your child's health record (due 12/30/11)
• Please remember to call the Save a Child Line (781-979-2192) when your child is absent.
It is most important to leave a detailed message, noting why your child
is absent and if they have a fever of 100 degrees or higher. Our school
policy states that students will not return to school until they are
fever, vomiting or diarrhea free for 24 hours. If their doctor ordered
an antibiotic for infections, they must have taken it for 24 hours
before returning to school. Any contagious infection such as strep
throat, conjunctivitis, or flu should be reported to the school nurse.
• If your child needs to receive any medications, including students who need inhalers or epi pens available, during school hours, call school nurse (781-665-6865) for consent forms or instructions.
• If your child has any orthopedic conditions such as a fracture or a sprain, please send in a doctor’s note stating their activity limitations, such as no recess or physical education.
• It is still important to have your family immunized against the flu. The H1N1 vaccine is included in the seasonal flu shot this year, so only one dose is necessary. • Vision and hearing screenings are done every year for all students in the elementary schools.
• As in the past school year, we will be measuring height and weight of all first and fourth graders. Informational letters will be sent to parents, along with results including BMI (Body Mass Index). A referral letter for your child’s pediatrician will be included if needed. You may excuse your child from this screening with a note to your school nurse.
• Grade 5 receives a scoliosis screening (with parental permission), and a growing up program in the spring. • Students entering Grade 3 in September are recommended to have a copy of a current physical for their school health record. These will be due by December.
• Any student entering kindergarten in September should have the required immunizations and a copy of a physical exam for his/her health record. Please see the school nurse with any questions.
• Encourage your children to play safe, remembering to wear sneakers for gym and recess, and dress appropriately for weather conditions.
