You have to fill out three originals, get signatures from your officers, and return it to city hall with the $50 fee. The permit should be available for you to pick up in a few days. Within a couple weeks or so you receive a package of raffle tax forms and envelopes from the state Lottery Commission. After you hold a raffle you have ten days to pay the 5% raffle tax on the gross revenue. The permit is good for a year and when it expires you have 30 days to submit an annual report showing your total raffle revenue and expenses for the permit year. The annual report is the second page of the raffle permit application so you can submit it with your permit renewal. PTOs may have as many raffles as they want each year.Right now, each PTO will have to pull its own gaming license; at this point in time, it would not be feasible for Melrose PTO Inc. officers to be responsible for completing the paperwork required. • We discussed the possibility of a district wide Lowell Devils fundraiser…we decided to let each PTO determine if they want to pursue it. • We discussed the possibility of staggering times for middle and elementary school holiday early releases, to make it easier for parents to pick up siblings. We also discussed the fact that high school students were released immediately after the Pep rally, which ended at 11:30, much to the surprise of the parents who came to pick them up at noon time. What is the district’s responsibility when releasing unsupervised teenagers early?
