REGISTRATION FORMS 2011-2012

Below we have provided registration forms for new volunteers, previously registered volunteers, and a form for those who wish to be listed in the Human Resource Directory.

PLEASE NOTE: All new volunteers must also complete The CORI Request Form. That form must be returned in person with a government issued photo ID such as a driver's license or passport It can be returned to the secretary at any Melrose Public School or to the volunteer office of the Bridge at 360 Lynn Fells Parkway.
